We would like to know your opinion on the services offered by the IRSST laboratories. This questionnaire focuses on three aspects of our services: equipment loans, sample collection and analysis, and our website. The information we collect will help us continuously improve the quality of our services.
This questionnaire is designed to be completed anonymously. No personally identifiable information is required to complete it. However, if you would like an IRSST representative to contact you to follow up on your comments, you may voluntarily provide your name and phone number in the sections provided for that purpose.
Any personal information you provide will be used solely to contact you regarding your comment or follow-up request. It will be treated confidentially and in accordance with applicable privacy laws.
The information collected through this questionnaire will be retained for a period of six (6) months from the date of receipt, after which it will be securely destroyed in accordance with the IRSST’s information management practices.
