Évaluation de l’exposition aux microorganismes et appréciation du risque sanitaire des chauffeurs de camion et des éboueurs attitrés au transport des matières résiduelles et des déchets <p>La gestion des déchets au Québec repose sur la collecte sélective pour revaloriser les matières résiduelles et réduire l’enfouissement. Les travailleurs du secteur sont exposés aux bioaérosols contenant microorganismes et allergènes. Les risques sanitaires sont influencés par la nature des matières manipulées et les méthodes de travail employées. La manutention des déchets a déjà été mise en lien avec des inflammations respiratoires, des troubles digestifs, et des syndromes liés aux poussières organiques toxiques. L’objectif du projet visait à évaluer l’exposition des travailleurs aux microorganismes et d’en apprécier les risques pour la santé.</p> <p>Dans le cadre de ce projet, 28 camions ont été étudiés : 12 pour les déchets ultimes, 10 les matières recyclables et 6 les résidus organiques. Des prélèvements d’air ont été effectués au niveau des voies respiratoires des travailleurs lors d’une journée type, ainsi que dans l’habitacle. Les filtres ont été collectés et le nombre de jours d’utilisation documenté. L’analyse a combiné culture microbiologique, biologie moléculaire, tests de cytotoxicité, évaluation de la résistance aux antimicrobiens et mesures de performance des filtres. Des essais en laboratoire ont également examiné l’effet de l’usure et du colmatage des filtres sur la circulation d’air, la réémission microbienne et la prolifération.</p> <p>Ce projet a démontré que <ul> <li>Les travailleurs affectés à la collecte des déchets sont exposés à des concentrations de bactéries et d’endotoxines pouvant dépasser les recommandations.</li> <li>Les éboueurs présentent les expositions les plus élevées en microorganismes cultivables.</li> <li>Des mycètes du groupe <em>Aspergillus</em> (section <em>Fumigati</em>), potentiellement pathogènes, ont été détectés dans la zone respiratoire lors de la décharge au site d’enfouissement.</li> <li>Les tests de cytotoxicité ont révélé un potentiel toxique des bioaérosols sur les cellules pulmonaires et hépatiques.</li> <li>Dans les filtres de cabine, plusieurs microorganismes présentant des risques sanitaires (<em>Aspergillus</em>, <em>E. coli</em>, <em>Enterobacter spp.</em>, <em>Legionella spp.</em>) ont été identifiés, avec des concentrations plus élevées dans les camions de matières organiques.</li> <li>L’efficacité de filtration mesurée variait largement (1,3 à 42 % pour la taille de particule la plus pénétrante).</li> <li>Aucun filtre n’offrait une efficacité élevée, et certains modèles présentaient une faible rétention microbienne associée à une filtration peu performante.</li> </ul></p> <p>Pour réduire l’exposition il est recommandé d’améliorer les méthodes de travail des éboueurs, notamment pour limiter le contact avec les matières organiques, d’installer des filtres d’habitacle plus performants et de renforcer les programmes d’entretien des cabines (remplacement des filtres, nettoyage des surfaces). Le présent rapport comprend le résumé de trois articles scientifiques qui abordent les résultats obtenus de cette étude.</p> <p><strong>Abstract</strong></p> <p>Waste management in Quebec is based on separate collection to recover residual materials and reduce landfill use. Workers in the sector are exposed to bioaerosols containing microorganisms and allergens. Health risks are affected by the nature of the materials handled and the work methods used. Waste handling has been found to be associated with respiratory inflammations, digestive disorders and organic dust toxic syndrome. The purpose of this project was to evaluate workers’ exposure to microorganisms and assess the risks for their health.</p> <p>During this project, 28 trucks were studied: 12 for ultimate wastes, 10 for recyclable materials and 6 for organic waste. Air samples were taken from the workers’ respiratory tracts during a typical day, as well as inside the cab. The filters were collected and the number of days of use was documented. The analysis combined microbiological culturing, molecular biology, cytotoxicity tests, evaluation of antimicrobial resistance and filter performance measurements. Laboratory tests also examined the effect of filter wear and clogging on air circulation, microbial retransmission and proliferation.</p> <p>This project proved that: <ul> <li>Workers assigned to waste collection are exposed to bacterial and endotoxin concentrations that can exceed recommended levels.</li> <li>Garbage collectors have the highest exposure to culturable microorganisms.</li> <li>Mycetes in the <em>Aspergillus</em> group (section <em>Fumigati</em>), which are potentially pathogenic, were detected in the breathing zone during unloading at the landfill site.</li> <li>Cytotoxicity tests revealed a potentially toxic effect of bioaerosols on lung and liver cells.</li> <li>In the cab filters, several microorganisms that present health risks (<em>Aspergillus</em>, <em>E. coli</em>, <em>Enterobacter spp.</em>, <em>Legionella spp.</em>) were identified, with higher concentrations in organic waste trucks.</li> <li>Filtration effectiveness measurements varied substantially (1.3% to 42% for the most penetrating particle size).</li> <li>None of the filters were highly effective and some models presented poor microbial retention associated with poor filtration.</li> </ul></p> <p>To reduce exposure, we recommend improving garbage collectors’ work methods, particularly to limit contact with organic materials, installing better-performing cab filters and enhancing cab maintenance programs (filter replacement, surface cleaning). Attention-enhanced U-net for robust N95 FFR segmentation in infrared imagery <p>Infrared (IR) imaging provides a non-invasive approach for evaluating the fit of N95 filtering facepiece respirators (N95 FFRs) and detecting potential leakage. This work presents an optimized U-Net architecture for N95 FFR segmentation from infrared images, a critical component for reliable leakage analysis. The proposed architecture integrates bilinear upsampling, Squeeze-and-Excitation (SE) blocks, and normalization techniques to improve feature representation, stabilize training, and reduce computational cost. The model is trained on a subset of participants and evaluated on separate validation and test partitions, composed of participants unseen during training and including mask types not encountered during that phase, enabling assessment of cross-participant generalization and robustness. Robust patch-level infrared leak detection on N95 respirators using spatiotemporal deep learning <p>Face seal leaks are a major cause of protection loss for N95 filtering facepiece respirators (N95 FFR), yet current fit-testing methods provide only a global fit factor and no information about leak location. Infrared (IR) imaging offers a way to visualise leaks by capturing temperature variations near the mask seal, but most existing approaches operate at the level of full-face images and do not explicitly model the temporal dynamics of breathing or the strong class imbalance between leak and non-leak samples. In this work, controlled infrared videos of N95 FFRs under calibrated leak scenarios, acquired on a respiratory test bench, are used to construct patch-level sequences centred on leak locations along the seal. A Bi-Level Class Balancing (BLCB) strategy is introduced, combining global resampling with targeted preservation of hard negative sequences. Three spatiotemporal deep architectures are evaluated, including two CNN-LSTM models and an attention-based model, all trained with a focal-type loss on balanced sequence sets. Experiments show that the attention-based model achieves high sequence-level performance (up to 98% accuracy and AUC-ROC close to 0.99 on several regions), while the BLCB scheme effectively mitigates class imbalance. Cross-device evaluation further indicates that models trained on one configuration maintain strong discriminative ability when transferred to the other, with only a modest drop in accuracy and F1-score. This paper presents an integrated framework leveraging infrared imaging and deep learning for real-time, non-contact leak localization. A custom U-Net model extracts the mask region from thermal images, while the Segment Anything Model 2 (SAM2) dynamically tracks contour variations under changing conditions. Thermal signals along the mask boundary are transformed into the frequency domain and correlated with a reference breathing signal to identify leak locations. The proposed method systematically evaluates optimal central region selection to enhance correlation-based leak detection. Assessing potential health risks from multi-exposure to chemicals in U.S. workplaces: findings from the OSHA database <p>Introduction</p> <p>The combined effects of occupational exposure to multiple chemicals on health can be substantial. However, the most prevalent multi-exposure situations and their toxic effects remain understudied. We assessed the health risks from multi-exposure to chemicals in U.S. workplaces using the Occupational Safety and Health Administration’s (OSHA) measurement database.</p> <p>Methods</p> <p>We analysed personal air measurements for the period 1971-2021 summarized by workplace situation (WS), corresponding to measurements taken for the same job title, within a company, within a year. We calculated hazard quotients (HQ) by dividing the agents’ concentrations by their ACGIH® threshold limit value. We calculated the mixtures’ hazard indices (HI) by summing the HQs of agents by combination of WS and toxicological class (n=24) using the MiXie tool, which identifies classes of toxic effects for >700 chemicals.</p> <p>Results</p> <p>We extracted 609,233 measurements of 206 chemicals from 162,473 WSs. Workers in 58,252 WSs were exposed to ≥2 agents, of which 21,563 had an HI>1, indicating overexposure for at least one toxicological class. Toxicological classes with the highest HIs among multi-exposed WSs were lower airway damage (median 0.41; interquartile interval 0.05-1.9; percentage of overexposed WSs 35%), ototoxicity (0.28; 0.04-1.1; 26%) and central nervous system (CNS) damage (0.26; 0.02-1.2; 28%). La pulvérisation de revêtements polyuréthanes contenant du diisocyanate d’hexaméthylène (HDI), sous forme monomère et d’oligomères constitue une source majeure d’exposition professionnelle, notamment dans les ateliers de carrosserie automobile. L’évaluation de l’exposition par inhalation à ces composés demeure toutefois complexe en raison de leur forte réactivité chimique et de leur présence simultanée sous forme de vapeur et de particules.</p> <p>La première phase de cette recherche visait à développer et valider un système de génération en laboratoire capable de reproduire de manière réaliste les conditions de pulvérisation de vernis transparents à base d’isocyanates.</p> <p>La deuxième phase consistait à utiliser ce système pour comparer trois méthodes de mesure d’isocyanates de type « filtre imprégné » (ISO 17734-1 avec dénudeur et méthodes avec cassette fermée 37 mm et cassette Swinnex 13 mm) à une méthode de référence utilisant un barboteur, basée sur la norme ISO 16702. Cette étude permettait d’examiner la performance des méthodes ainsi que la répartition spatiale des isocyanates dans les dispositifs d’échantillonnage. Des analyses par microscopie électronique à balayage (MEB) ont également été réalisées afin d’observer l’accumulation de vernis sur les filtres. Les résultats ont montré que, certaines sous-sections d’échantillonneurs captaient préférentiellement le HDI (vapeur) ou l’isocyanurate, principal oligomère (aérosol liquide). Aucune différence significative n’a été observée entre les méthodes testées et la méthode de référence, bien que la variabilité des résultats fût élevée.</p> <p>La troisième phase de l’étude a porté sur l’application de cette comparaison dans un atelier de carrosserie automobile. Dans ce contexte, les aérosols générées étaient plus grossiers, et la répartition spatiale du HDI et de l’isocyanurate dans les dispositifs présentait des tendances similaires à celles observées en laboratoire. Les observations par MEB ont mis en évidence une accumulation importante de gouttelettes de vernis sur les filtres, susceptible de limiter l’efficacité des réactions chimiques nécessaires à l’analyse. Les trois méthodes testées ont mené à une sous-estimation des concentrations pour les isocyanurates, suggérant un lien entre la vitesse de réaction de l’activateur du vernis et le biais observé par rapport à la méthode de référence.</p> <p>Cette étude souligne la complémentarité des essais en laboratoire et sur le terrain pour évaluer les limites des méthodes de mesure des isocyanates. Elle met en évidence l’importance du choix du dispositif d’échantillonnage, de la prise en compte de la réactivité chimique des composés afin d’obtenir des résultats fiables, et met en garde contre une dépendance exclusive aux méthodes sur filtre dans des contextes impliquant des particules réactives. Ce rapport comprend le résumé des articles publiés abordant les résultats de l’étude.</p> <p>Abstract</p> <p>Isocyanates are irritant compounds that induce respiratory sensitization and occupational asthma. The atomization of polyurethane coatings containing hexamethylene diisocyanate (HDI), in monomer and oligomer form, constitutes a major source of occupational exposure, particularly in automobile body shops. However, the assessment of inhalation exposure to these compounds is still complex because of their strong chemical reactivity and the fact that they exist simultaneously as fumes and as particles.</p> <p>The first phase of this research was planned to develop and validate a laboratory generation system capable of realistically reproducing the conditions for atomizing transparent isocyanate-based varnishes.</p> <p>The second phase consisted in using this system to compare three “impregnated filter” type methods for measuring isocyanates (ISO 17734-1 with an air stripper, and methods with a closed 37 mm cassette and a Swinnex 13 mm cassette) to a reference method using a bubbler, based on ISO 16702. This study examined the methods’ performance and the spatial distribution of isocyanates in the sampling devices. Scanning electron microscopy (SEM) analyses were also done to observe the accumulation of varnish on the filters. The results showed that certain subsections of the sampling devices preferentially collected HDI (fumes) or isocyanurate, the main oligomer (liquid aerosol). No significant difference was observed between the test methods and the reference method, although the results were highly variable.</p> <p>The third phase of the study concerned the application of this comparison in an auto body shop. In this context, the aerosols generated were coarser and the spatial distribution of HDI and isocyanurate in the devices showed similar trends to those observed in the laboratory. The SEM observations revealed a substantial accumulation of varnish droplets on the filters, which could limit the effectiveness of the chemical reactions required for analysis. All three test methods underestimated the isocyanurate concentrations, suggesting that there is a link between the varnish activator’s reaction speed and the bias observed in relation to the reference method.</p> <p>This study highlights the complementarity of testing in the lab and on the ground to assess the limits of isocyanates measurement methods. It underscores the importance of the choice of sampling device and of taking the compounds’ chemical reactivity into account to obtain reliable results. It also cautions users against depending exclusively on filter-based methods in contexts involving reactive particles. Are newly graduated physiotherapists in Quebec prepared for work rehabilitation practice? A mixed method study <p><h3>Purpose</h3></p> <p>Physiotherapists are frontline providers in supporting the return-to-work process of individuals with musculoskeletal disorders. However, many report feeling unprepared for work rehabilitation following entry-level training. This study explored recent physiotherapy graduates in Quebec, Canada, perceived preparedness to practice in work rehabilitation and its influencing factors. <h3>Materials and Methods</h3></p> <p>A convergent mixed methods design grounded in a competency-based framework was used. Recent graduates from physiotherapy programs completed a cross-sectional survey rating their perceived preparedness across seven work rehabilitation competencies. Semi-structured individual interviews explored how their training prepared them for work rehabilitation practice. Quantitative data were analyzed descriptively, and interviews were thematically analyzed. Findings were integrated through a joint display to contextualize preparedness ratings with qualitative insights. <h3>Results</h3></p> <p>Twenty-five recent graduates from five physiotherapy programs across the province of Quebec completed both the survey and the interview. Perceived preparedness was highest for competencies on person-centered care and collaborative treatment planning, and lowest for psychosocial factors management, collaboration with involved actors, compensation system navigation, and return-to-work support. Three overarching themes influenced perceived preparedness: (1) role perceptions in work rehabilitation, (2) enablers such as supportive curriculum elements, and (3) challenges including stigma toward injured workers and limited work rehabilitation-specific training. A fourth theme described strategies to improve work rehabilitation training. <h3>Conclusions</h3></p> <p>Recent physiotherapy graduates reported varying levels of perceived preparedness for work rehabilitation, with the lowest ratings associated with managing psychosocial factors, collaborating with involved actors, navigating compensation systems, and supporting return to work. These gaps were linked to limited work rehabilitation training, perceived role ambiguity, and exposure to stigma. Barrier face coverings: Impact of washing on filtration efficiency using viral model and submicron particles <p>Barrier face coverings were commonly used during the SARS-CoV-2 pandemic as a solution for the shortage of medical-grade masks. Many of these devices are designed to be washable and reusable. As concerns rise about the environmental repercussions of single-use masks, the use of efficient, reusable barrier face coverings has begun to emerge as an alternative. If the ASTM standard F3502-21 governs the efficiency of inert particle filtration, it does not, however, require that devices be tested against viral aerosols. Although the impact of repeated washes on these devices’ ability to filter inert particles has been investigated, disagreements remain on whether washing barrier face coverings can significantly affect their efficiency. The goal of this study was to evaluate the impact of washes on the filtration efficiency of five barrier face coverings from a single manufacturer against submicron particles and a model virus. To that end, a wind tunnel specifically designed for the evaluation of masks’ filtration efficiency was used in conjunction with bacteriophage PhiX174, which proxied for human pathogens. The five barrier face coverings were washed and tested at different washing frequencies, determined as per the manufacturer’s indications. Results showed a significant loss of efficiency in filtering submicron particles and viruses for all five barrier face coverings. Resistance to washing varied across the tested devices. The aim of the present study was to monitor the expression of Vim, ENO1, and their citrullinated forms (cit-Vim, cit-ENO1) during the regulation of spontaneous human neutrophil apoptosis (SA) by the antiapoptotic cytokine granulocyte–macrophage colony-stimulating factor (GM-CSF) and the proapoptotic anti-cancer drug arsenic trioxide (ATO). Although the degradation of Vim during SA was delayed by GM-CSF and accelerated by ATO, no significant degradation of cit-Vim, ENO1, and cit-ENO1 was observed. However, Vim, ENO1, cit-Vim, and cit-ENO1 are expressed at the cell surface of apoptotic neutrophils. Occupational exposure to endocrine-disrupting chemicals and colorectal cancer risk: An analysis of four participating cohorts of the Canadian Partnership for Tomorrow's Health study <p><h3 id="x-x-sectitle0015">Introduction</h3> Endocrine-disrupting chemicals (EDCs) may influence cancer risk by interfering with sex hormone balance. Yet, the role of EDCs in colorectal cancer remains underexplored. EDC exposure is common in the environment, but levels are much higher in certain workplaces. This study assessed the association between occupational exposure to selected EDCs and colorectal cancer risk. <h3 id="x-x-sectitle0020">Methods</h3> A case-cohort study was nested within four regional cohorts of the Canadian Partnership for Tomorrow's Health study. Cases included 1083 incident colorectal cancer cases, and a subcohort of 4788 participants was selected at baseline. Occupational exposure to 17 EDCs in participants' longest-held job was estimated using CANJEM, a job exposure matrix, which provided the probability of exposure for each EDC-job combination. Participants were categorized into unexposed (probability = 0), possibly exposed (0 <h3 id="x-x-sectitle0025">Results</h3> Compared to unexposed participants, participants exposed to antiestrogenic EDCs had an elevated colorectal cancer risk (OR = 1.31, 95%CI: 1.01–1.70). Risk was increased for possible exposure to arsenic (OR = 1.67, 95%CI: 1.27–2.21) and polychlorinated biphenyls (OR = 3.34, 95%CI: 2.20–5.07). Participants possibly exposed to cadmium (OR = 0.78, 95%CI: 0.63-0.96) had a lower colorectal cancer risk. <h3 id="x-x-sectitle0030">Conclusion</h3> We observed some associations between occupational EDC exposure and colorectal cancer risk. Relationships between magnitude of handedness and bilateral asymmetry of shoulder muscle activation during a unilateral overhead fatiguing task in right-handed individuals <p>Previous literature has shown a relationship between side dominance and asymmetry in some features of motor control. However, the magnitude of handedness (i.e. how side-dominant someone is) is unclearly related to bilateral differences in shoulder muscle activity and how it changes with fatigue. We expected that people with the highest scores on the handedness laterality quotient would display a greater side dominance (difference between their right and left sides) in their shoulder activation patterns, both in the absence and presence of fatigue. Thirty right-handed adults completed two sessions of an overhead fatiguing task: one with their dominant and the other with their non-dominant arms. Electromyographic (EMG) data was collected from six shoulder muscles (deltoids, rotator cuff and biceps) using either surface and in-dwelling electrodes, and activation amplitude was calculated using root-mean-square (RMS). Asymmetry was calculated using a Limb Symmetry Index equation. Two statistically significant correlations were found, only in females, between the handedness laterality quotient (degree of right-handedness) with no-fatigue anterior deltoid EMG RMS asymmetry (p = 0.004), and with fatigued middle deltoid EMG RMS asymmetry (p = 0.006). Handedness magnitude should be considered when recommending shoulder injury prevention approaches for overhead work, but only in females. Assessing integrative prevention at work: A scoping review <p><h3>Background</h3></p> <p>Integrative prevention at work is a promising avenue to better prevent occupational injuries and manage prolonged incapacity in a changing world of work. Integrative prevention at work can be operationalized using its five defining attributes: (1) holistic vision of health, (2) common understanding of the purpose of integrative prevention, (3) communication among stakeholders, (4) collaboration among stakeholders, and (5) coordination of preventive action. An assessment tool for these characteristics would be a valuable resource for organizations seeking to improve their approach to prevention. Namely, it would allow organizations to assess the presence of integrative prevention at work in their environment and enhance their ability to implement it. <h3>Objective</h3></p> <p>This study aimed to describe the evaluation tools assessing attributes of integrative prevention at work. <h3>Methods</h3></p> <p>This scoping review followed a five-step process: 1) identifying the research question, 2) identifying relevant documents, 3) selecting documents, 4) extracting the data, and 5) examining, synthesizing, and reporting the results. <h3>Results</h3></p> <p>Twelve evaluation tools were identified assessing one or more attributes of integrative prevention at work. Descriptive elements are provided for each tool (e.g., its purpose, the attribute(s) it assesses, and its metrological properties). Our study suggests that communication among stakeholders and collaboration among stakeholders are the attributes that are the most assessed by the evaluation tools. <h3>Conclusions</h3></p> <p>This study provides the first comprehensive and detailed overview of the extant tools currently being used to assess the attributes of integrative prevention at work.
